First Office Visit

We ask that our patients either fill out the new patient paperwork prior to showing up to their appointment or to show up 15-20 minutes early to ensure time to fill out all paperwork prior to the start of visit. You will recieve the paperwork in an e-mail prior to your arrival. Even if you have completed all of the paperwork prior to your arrival please show up at least 10 minutes prior to the start of your visit to ensure you have adequate time to sign the consent forms.

Please bring any relevant medical records along with you to our first visit. This will ensure that we can get started as quickly as possible on a treatment plan that reflects everything we can know about your medical state up until the end of your first visit. If you do not have access to your medical records we can fill out a medical records release form in office. It can take up to 6 weeks to recieve all relevant medical information when we send out these requests so please plan accordingly.

We recommend that you activate your patient portal as soon as possible so that you can send your medical records to the physician prior to your visit through the HIPPA compliant secure messaging system. It is often helpful for the physician to review any forms or medical records prior to the visit.

Telemedicine

We understand that not everyone is able to attend their appointments in person. Whether you are vacationing in a cabin in the woods or you are at home, if you are within wi-fi range it is no problem. There are some medical conditions that are better served in person, than via telemedicine. If you have a question as whether your concern is one of those do not hesitate to reach out to your care provider.
